PAYMENT:
A ten percent non-refundable deposit is required to book a surgery date. The balance of your fee is due in full three weeks prior to your surgery date. Occasionally we alter the surgery schedule to accommodate patients within the three weeks - in those cases the full surgical fee is due before we can make any changes in the schedule. Deposits are non-refundable but if due to an unforeseen event, cancellation is made more than three weeks before surgery your deposit will be transferable to another date within five months. Please call our office about the available payment plans.

CANCELLATION:
Cancellation within three weeks of the surgery date cannot be refunded unless Dr. Loeb is able to use that time for another patient. In cases when you forfeit your fee Dr. Loeb will credit half of your fee to another surgery date within five months. Anesthesia and O.R. charges are fully refunded. Cancellations within forty-eight hours of surgery receive no refund at all.

If your surgery is canceled for documented medical reasons we will apply payments towards a future date (within five months).
